Guess the Abnormal Finding.

This patient presented with altered mental status and fever. An MRI of the brain showed the above results. What is the next appropriate treatment or test?

    This is a brain abscess with mass effect moving the middle line, the patients needs surgery, for aspiration and biopsy. An optimal treatment approach has not been established. However, there is evidence that the size of the lesion and clinical and immune status of the patient are relevant to surgical decision making. Earlier biopsy of lesions to achieve specific identification and anti-microbial sensitivity profiles is suggested even in cases where an extracranial focus of infection is found.
